Dr. Blaize was born in Port Allen, LA. He graduated from St. Joseph Seminary College and LSU Medical Center in New Orleans. Upon completion of his internship and residency in internal medicine at UTMB in Galveston, TX, Dr. Blaize returned to Louisiana in 1983 and started his practice in Baton Rouge in internal medicine. Dr. Blaize is board certified in internal medicine. He is a member of the Louisiana State Medical Society, the American College of Physicians and the American Society of Internal Medicine. Dr. Blaize specializes in adult primary care and preventive medicine in both the hospital and office settings.
